Cancellation Policy

• 48-hour notice required for waived fee

• $100 fee automatically charged to account

• 15+ minutes late for appointment requires reschedule and cancellation fee applies

Payment Policy

• Copays, deductibles, and non-insurance charges are due at time of visit

• Payments 90 days past due will be flagged, patient will be contacted

• Payments 110+ days past due are sent to collections
